Please make sure you select your birth information carefully. Click on the birth date box and use the calendar to select your birth date. If you choose to type the date in, it should be entered in the American format of  MONTH/DAY/YEAR.  For example, if you were born February 10, 1965, your birth date would read numerically as 02/10/1965. Use 2 digits for the month, 2 digits for the day, and 4 digits for the year.
